had a busy day today...
3 out of 8 MO today was off today....
So, a bit short of staff here, especially in polyclinic...
Finished seeing patient only at 5.30pm today...
Then came back to Hospital have to start oncall....

Just settled all the case now....
Hope later today (Wednesday) polyclinic will have less patients....

Well, January month is my 2 best buddies bday month....
One is today, 4th January, another is 2 days later on 6th January...

Memories during our first year when we celebrated the birthday as a group....

I dont have any Bryan's birthday photos, coz he hates photo...

It's Christmas today! But still have to work like usual...
And good thing is i m not oncall today!

After finished the O+G round today, went to help my friend at Medical ward...
Just when i stepped into the ward, my friend told me that one of the PTB patient in the ward wasnt good and may need intubation! Damn! If he's intubated, I'm the only one who's available to escort him down to SIbu......No...I dont wanna spend my christmas night in Hosp Sibu....But in the end, the patient's family member decided not to send the patient down, so i'm save from escorting the patient!

Tonight's plans will be either house visiting or attending church service...

And i chose house visitng....

Back at home at 10.30pm with bloated tummy....

My housemate went back to kuching today, so left me alone in the house again..
Today's Thursday, i was in MCH, seeing and scanning all the pregnant lady...
After MCH finished at 5pm, I had to present my mortality review on a PTB patient...
Presentation finished at 7pm...

Proceeded to my barrack, with no lights on...
Cause no one is back to the house yet...cause my housemate was on his way to kuching..and i'll be alone tonight! huahaha....

Day 62 - Lost but Found...

Day 62 in UK....
Just came back from holidays 2 days ago....
Working mood has slowly settling now....
Was oncall on Friday....
Here's my lunch on my oncall day....So much that i had to force myself to finish it..
haha..

Lost but found, my dearest watch....

I accidentally left it in my OT clothes yesterday after I did an ERPOC...
Only realized that i left it there when i was on my way to SIbu...
Prayed that the watch was still in the OT clothes...If not, i have to find a way to replace the watch....but such an expensive watch, who am i going to ask for help?

Once i reached Sibu Hospital, I called up the PBK oncall to help me look for it...
The PBK was kind enough to search from the OT clothes and passed it to ER nurse..
And i claimed from the nurse once i back fr Sibu...
Phew...Thank God i got it back..if not, half of my monthly salary will have gone...
Got to be really careful next time....

Dell Inspiron, my precious computer is back with SOUND after one week of MUTE!!
Had miserable time with it MUTED for dunno what reason!
Thanks to QQ service centre for his expert management ! Hahaha...

4 hours... After 4 hours, i'll be in the express boat towards UK...
Having insomnia now....

Like my friend said, why do all good things come to an end?
Well, bad things will also come to an end,
But always, we prefer the good things to stay static forever...
Example... our holidays~
Sigh....

5 days of leaves were really short....
But still thank God that through a good friend's wedding that He brought everyone of us together after 2+ years...
Had lots of fun and laughter together during the gathering...
And now, everyone had gone back to their own life including myself...
When will be our next gathering?
What kind of event that will bring all of us altogether again?

Christmas time is in the air..

Christmas is around the corner......
Miss the white christmas during my university time......
Sigh..miss the snow...miss the christmas decoration there....miss the fun time playing with the snow...miss the time having fun with bunch of friends and groupmates....
Miss those old time very very much!

As i mentioned in previous post, i'll be escorting a patient by medivac 2nd time, but unfortunately the medivac can't fly to UK cause they need to bring a patient from bintulu to kuching. So, I escorted the patient by ambulance boat for the first time. THe boat will stop at Kanowit, 2 hour ride to kanowit, then from Kanowit, the Sibu ambulance will bring us to Hospital Sibu which will be another 40 minutes ride.

I thought I will be escorting a patient, but in the end, i escorted another intubated Pulmonary TB patient, who deteriorated early in the morning.

This is the ambulance boat...small boat....leave at 12 pm...

Inside the boat...the adult intubated patient was put at the middle there, and the prem baby was carried by the staff nurse and sit at the side....
Ambulance boat was really a bad transport to transfer a patient, cause it kept bumping up and down...no wonder we have to stop at Kanowit to use the ambulance to Hospital Sibu...

Reached at Kanowit at 2 pm...

Reach at hospital sibu at 2.45 pm.....so had to stay in Sibu overnight again this time....Thank God, Josephine worked at office hour, so she was free to accompany me...

During this weekend, got one VVIP came to UK, to celebrate Maal Hijrah here...
And he came at the wrong time, cause here's having a 'measles season'...
Thus, 2 medical teams was summoned to do their job, one team for measles screening to prevent the suspicious sick person to go near him, another team to stand by in case anything happen to him....

It's been more than a month here in UK....
Life has been peaceful and steady....not as busy as the big city hospital though...

During this 1 month, first i started in polyclinic, then i helped covered medical ward for 1 week....Medical ward really is the most busy ward in any hospital....
then after 1 week of hectic and busy work, I m back to polyclinic again currently...

Tomorrow will be my second time medivac a 32 wk prem again early in the morning to Hospital SIbu.....and have to rush back to hospital again in the afternoon....

Day 1 (18/10/11) - the day had finally come after much delayed ....
Flight to Sibu at 6.10 am ... 35 minutes

Then express boat to UK.. 2 hours and 15 minutes....

After 1 hour...all you can see is jungle.......

On arrival at Kapit port wharf.... got a sms from the Senior, saying that he will not be around for today, cause he has to go to Sibu for something...
First question: Ada taxi kah? Answer: Tak de taxi kat sini, ada van la...
Second question: Mana hospital Kapit? Answer: Oh, dekat saja, jalan pergi sana, nampak satu kolam besar, kat sana saja....
okay, then we just walk and walk, then asked around again and again, finally, after 10 minutes walk, here it is...Kapit Hospital...

Continue walking, saw one Matron with a bunch of nurses just came out from seminar room.....
Us: Erm...we are the new dr from SGH..
Matron: Oh....whoa, kamu orang jalan kaki bawa bag bag besar ini?
Us: Ya, they said hospital is very near, so we just walk from the port wharf...
Matron: Kesian.....ikut saya....

Followed her to Admin office to settle some admin paper work...
After settle some part of it, we were wondering where are we going to stay here?
I thought i will be staying with a sister, that's the plan in the beginning before i came here...then matron said, the sister has family who will come once in 2 weeks, so it's a bit inconvenient for me and for her...
But there's an empty quarter here (the only empty one) but then it's not ready for stay yet...cause she heard from the rumors that we will be coming in November, thats why she didnt prepare early...
For my friend law, he will be staying in the other quarter with others male officer..

Then she show us the quarter, a whole quarter for myself only...

Thats my quarter, up at the hill there....only the left side is ours...the right half is belong to a nurse and her family....
There's a car, belongs to the neighbor..

Then turn right, there's a small passage way to 2 rooms...

That's everything for my quarter....quarter looks large for a person, so i asked my friend to stay in with me, so that we can share for everything thats needed..

There's another quarter opposite ours..its occupied..

After the 'tour', our feeling is same as the feel when we first arrive at Kursk, and it's even worse than kursk... =(

Then sister brought me to her house, to stay for tonight only, with the hope that the quarter will be ready tomorrow.... My friend will be staying at surgical ward isolation ward...

Now, the surgical ward, first ward we visited...because my friend staying here for tonight...

After the dinner, we had a walk round the kapit town, finished in 2 hours time...
Very small town....

Then walk back to sister's quarter, slept at 9.30pm, cause there's nothing to do here.... - The end of Day 1 -

Day 2- woke up at 7.30am, sister prepared a cereal drink for me, then met with my friend at the admin office, to settle the unsettle admin work, to get the chop done, then orientation with pathology department, x-ray department, pharmacy, and etc..

Also have to reconnect back the electricity and the water supply for our new quarter, at sesco and JKR with the help of driver......
The most important thing, the streamyx....TM point is very near to hospital, so the first thing we did before sesco and JKR was to apply the streamyx...
The TM point officer said he has to survey our area first before he can install the streamyx for us....that will be done by tomorrow....

By evening, the matron informed us that the quarter was ready for us already, well, she kept her promise though that it will be finished by today...

And also i realized my room is a bit cooler in the afternoon compared to the other room...Kapit's afternoon is so damn hot but it's really cool at night...

Things i bought today (Day 2)

Day 3- woke up a bit later at 8 am....
Wanted to go to admin to settle my claim, but the admin officer wasnt there, guess where he MIA at 9am? He went to 'minum' with some chief.. WTH?????!!
We went to see him yesterday at 8am, he's on his way ...
Today, 9 am, 'minum'.... *speechless*

So we decided to go out to the town again to buy our stuff...
On the way out, we stopped by at TM point again, to bug them again, cause my friend and i both want the streamyx to be installed real fast because we both cant live without internet....They said they will get it As soon as possible,and so, i asked who's the guy will come to survey, and his phone number. I called him up myself, and he said he will come in the evening....ok...lets see if he comes or not..

Things we bought today, Washing machine...RM400

Kettle (rm 56) and rice cooker (rm 104).....

By 3pm, the TM point people came to install the phone line...which cost us RM 85..
then he said there will be another person coming later in the evening to install our internet.....i look at my watch, it's 5.15pm already, so i asked him again, you sure he will be coming later? then he said 'sure, coz they gave priority to us, and they are actually contract from telekom, not the telekom own workers..'
hmm... did i push them too much...?

Yeappie! Streamyx installed and ready to use at 7 pm! Cool!
And see, it's D-Link! wow! The person said previously when they used the other lousy brand, the customer complaint to them after 4 days....cause any problem, we will look for that person, not the telekom, that's why they requested for D-link router..

TOday, my friend blocked the bathroom cause he washed all the dirt sediments down to the drain and blocked d drainage system until the water come out from the kitchen drain.... but was later repaired by the Faber ...

Now left one thing hasnt settle - our claims.....
will be starting our work on Monday in polyclinic first....
Maybe we'll go to the polyclinic to have a look tomorrow evening...
For the time being, there'll be 8 MOs including us
Chong, Voon, HO, Abu, Hisyam, Kiu, and both of us
Will have one call this month...damn....
Heard sometime it's very free, but sometime it's real busy.....
Every complicated cases or unsure cases, all refer to Sibu Specialists...
Some specialists are nice, especially the Paeds HOD, Dr. Wong... heard he even answer the calls and take the referral eventhough he was on leave...

The list A drugs here have to be approved by physician first before the pharmacist dispense the drug, so any patient need list A drug, we have to call up the physician..

Application for transfer can only be made after one year of service here, if not, they wont 'layan' your application..

Hairdo day & Countdown - 5 days!!!

Enjoying my holidays currently....
Today, Day 2 of my leave, didnt do much, spent some time to organize my laptop and also helped taking care of my nephew. Then, looking for friends to get a hairdo together and going out for dinner tomorrow. Went to SIMC to get something from a MA then met my friend there. Coincidentally, she also planning to get her perm hairdo, which i wanted to do oso, so we decided to go after she off from work at 3pm....

Here's my first and the most expensive hairdo in my 26 years of life...
Cost me RM 342 (after 10% off of RM380= 180 (color)+ 200(digital perm)
We did it in Crowning Glory, a hair salon just above the Watson's store at Jalan Song. Well, the stylist was quite nice, he chose the color for me...
Didn't plan to color my never-been-colored-hair...but my friend kept saying that perm hair need to come with color only nice....so ok lor....color lor...

God's call ? or not?

On 26th August 2011, morning 8.45am, on my way back to A+E department from my weekly FridaY CME, my friend called me from back,
"Hey!! Bry!! I got a news for you!! You're going to Kapit!!"
I was like "huh???!?!?! Your head la!! Dont joke man! I dont believe you! Dont you try to bluff me!"
My friend replied "Really!! Come come, I let you see the list" while she's busy opening up the photo of the list she took but her phone was lagging!...
I still couldn't trust her that I am going to Kapit!!
So before i could start my work peacefully, i went to the board with my friend and see it with my own eyes....and true enough, my name
"Bry" - HUS Kuching --> HOS Kapit --> Tarikh kuatkuasa 19.10.2011

Oh my goodness...it's real!! How can it be so fast!! I thought i will only know like one month before the end of my HO? And yet, some of my batchmates still dont know where are they going!!

Sigh, it's Kapit!! It's so far away, and so ulu!! Why me? Why am I always stick together with my friend Law? Why?

Everyone in A+E were so excited for me that I am going to Kapit. And me was asking around,
"If you were me, will you wanna appeal ?"
Some said, NO, some said YES....
No, because there will be risk for them going to Kapit if there's a vacancy there...
Yes, because they think it's challenging and adventurous....

And me, Yes? No? Yes appeal? No appeal??
Is that God's call? If GOd's call, why Kapit?
He has something big plans there for me? He wants me to be out of my comfort zone? out of my own small box? Can I not follow His call?
Or is it because I haven't pray hard enough for not going so far away?

I remembered I had never prayed for not going to Kapit, instead i prayed that I don't want to stay in SGH and I would like to go to Bintulu which is nearer to Kuching, but I added on in my prayer that, the final decision is still on His hands, and I know His plans will be the best for me....
I only prayed that prayer once.... and after that i am sort of neglected Him except going for church service or when i had hard time.....
Is Kapit one of the way to pull me back to God, because there, I will have to be independent, and the only one who I can really depend on is my God....

During my lunch break, I went to see the list again, and also to 'ge bo' others colleague postings,suddenly Michael appeared, " You dont need to see the list again and again la! No matter how many times you see, your name is in Kapit already la!!! hahaha.."
Damn, another stab wound on my heart....hahaha..

More and more questions in my mind......which then cause me a tension headache on my way back home....cause I still cant believe me is going to a place, one hour flight to Sibu then another 3 hours express boat!!

At home, another flow of suggestions, from my mother, dad, brother,
"Dont go to Kapit, go appeal, Kapit too far away!!" "Dont work so hard anymore, try to get extended in your last department then others will fill up your vacancy cause i will be not on time to go to Kapit....try to be more "rebellion"

If i really want to appeal, i had to have a really valid and good reason...but do i have one? NO!
Family? I need to take care of my mother? Will it be that easy since the list is out already!
Try to get extended? Will that influence my future career later on?
Sigh.............. TOok a declofenac and went to sleep early cause i dont wanna think of it again...
Day 26th August 2011 ended with a declofenac and an early dream...

Day 27th August 2011 started at 7.30am, like usual, go for teaching then back to work for pm shift.....
Slowly trying to accept the fact that i m going to Kapit a/w LOA.....
Having palpitation almost throughout the day, worry if I see that kind of case, how would i manage it correctly? How would i diagnose as? If i cant tube a patient, what am I gonna do? So many 'HowS' and 'WhatS'! No specialists there to guide me in Kapit....no one! Suddenly, i felt like i dont know anything! WOrries and worries....
Current time 2.05am, and me still alert like nobody business cause i m having
Insomnia secondary to Kapit.....

Sigh....got to sleep now, having double shift later today.....

A 6 years old little boy, loves his father dearly.
The two have little time together because when the boy went to school, the father was still sleeping.
And when father came back from work, the boy was already in the bed.

One weekend, the two were together at home,
The son asked the father,
"Dad, can you give me $10?"
Dad answered harshly "why do you need $10?! I work so hard and provides everything that you need. Why do you still need the $10!! Dont waste! Go back to your room."
Tears rolled down on boy's cheek and ran into the room.

A while, the father then felt guilty and went to boy's room, sat down and apologized
"Son, sorry that I've been harsh to you just now. Can you tell me why do you need the $10? What are you going to do with it?"
Son opened up his drawer and showed to his father,
"Dad, I have saved this much of money, $10, and I need another $10 to have an hour of yours (Father works $20 per hour)"
Father then broke down...

The son, is like our God, He loves us so much that He gave His own Son, Jesus Christ to us, hope we all will be with Him forever and ever.
The father is like us, we thought we have provides everything to God by serving Him day in day out, but we never have a silent moment, to just sit down and be with Him.

Serving God is important, but spending time with God is also important...

Home Hydration

Just came back from my double shift. Wasn't a bad night though. But felt bad for my friend cause I slept from 6am till 7.30am and she's still seeing patient who came in one after another...

Had experienced a very bad sleep yesterday evening before work, cause I have to wake up hourly to drink due to a drying up sorethroat. So i decided to hydrate myself at home, by setting myself a line and running fluid my own....But where should i set it...
My friend offered to set for me on hand but if i set it on hand, how can i change or put up drip myself....
SO, best place to set is on your leg......and I used the smallest branula, the kids branula.....
First attempt on left foot, failed...
Second attempt on Right foot, succeed ......Phew....If not, I gonna bear the hourly self oral hydration....

Well, the hydration was effective enough that i was able to sleep through a good 5 hours sleep....and woke up with no more sorethroat.....Huahaha.....TOtal hydration-5 pints...

Am double shift-ing now. Currently patient ZERO.
Managed to clear the piled up green zone cases by 2 am. The MO on night was good. He helped seeing green zone cases also and all the 3 yellow zone HO including me also helped out. That's why cases were cleared so fast.

4 mOre hours tO go to finish my double shift.

Josephine coming on next Tuesday for interview but too bad my next week schedule not out yet,so don't know how free am I to accompany her.

Had just finished the last episode of City Hunter Korean Series today. Though it's not a bad ending but still, I'll miss the series because of Lee Min Ho and also Kim Na Na. Those two actor and actress can really act, truly a professional. Sigh ...
Missing them. Hope there'll be City Hunter Part Two or a special episode also can.

What else to write about and babbling at this time?
Too bad I can't upload the picture from my iPhone to blogger.
Battery now only left 26%!!! Time to look for my friend for the charger.

Double shift today again....nowadays will have more frequent double shift because more and more HO gonna graduate and leaving for district hospitals....
But, it's alright, because the more you have double shift, the more sleeping day you get...haha.... At least 2 sleeping days a week.....

As usual, today got teaching in the morning, starting at 9am....
Presentation topics today were Regional block, HF, Tetanus....

Had multiple episodes of sleepiness attack during the whole teaching session...
In order to keep myself more alert, I played around with my phone camera and took a photo of my working shoe named ECCO...

Was chit-chatting with my friend yesterday when I kind of teased her of buying new shoes again....which cost her RM 40, which sound expensive to me....Then she teased me back that my shoe was much more expensive than hers, around 5 times of her shoe...
Well, I know my shoe was not cheap, but didnt know it was that expensive, cause my dad bought it for me and he said that time got discount so he said it's cheap.....
Never thought that, there's actually someone noticed my shoe...haha...
Search some info regarding my shoe on internet after being teased by my friend....
Surprisingly, there are many more nice style of shoes ECCO produced.....such as:

Okay, enough for my shoe....Back to today's teaching which was good for double shift and morning shift ppl cause it lasted till 1 pm, then only we started to work..haha..
SO meaning, I only work for 2 hours then will have the 6 hours break before my night shift...whereas the morning shift will only need to work for 3 hours...but it's bad for PM shift, cause they will have to come back to work at 3 pm...

Okay...time for me to sleep now.......hope tonight's shift will be good.....

Planned to have dinner with Mervyn, Wong KY and QQ but only 2 turned up,
because QQ "放飞机"

We ordered this "Zaigle Fusion- Large" -RM68.... for 4 ppl portion but just enough for 3 of us.....

That comes with 2 free refillable side dishes and leaves....
Total time of leaves we refilled...7 times...haha....

Post night aka Post call today.....So happy cause got whole day for me to sleep...
Wasn't a bad call...thank God...haha...

Started with a good morning shift.....

0 customers...and that time was...

10.30 am

Until 11am, came one patient and subsequently more and more patients came in toward the afternoon...sigh....

Then started night shift with a whole bunch of hyperventilation young girl, total 6 patients, from the same secondary school and from the same place....
Apparently they were having a fire drill session, and dont know how they all got hypervent ...........

Anyway, towards the midnight, less and less patients coming in to yellow zone, so as usual, some of us went to green zone to clear all the cases ASAP, around 2 am, all the cases cleared.....but still there will be on and off one or two cases coming in....

Went for free morning breakfast at 7.30am....Sandwiches!

8 am......Another double shift ended wonderfully.....Thank God...

Yea....no Morning teaching session today...so don't have to wake up in the morning for 8am teaching session. Just sleep till late morning and wait for PM shift at 3 pm....

It's nice to wake up naturally in the late morning.
Actually Morning teaching session is not a bad thing though........
We have this session 3 times a week, wednesday, thursday and Saturday, all started by an ED specialist Dr. Ch. But this weekend he's flying to Miri so this Saturday's teaching session was canceled.

Everytime during teaching session, we all get to discuss many interesting cases, and also about what can we miss out in a patient came in with just a simple complain.
Also ED specialists sharing their old times experiences and stories.

The last teaching session, we were discussing about antivenom. Why do we have delay in antivenom treatment sometime? What's the factors?
Our experienced ED HOD, told us by his experience and came out with 4 factors:

1) Unaware of what happen to the patient as patient came in unconscious....All the vitals crushed down, without knowing the source, then was sent to the ward, noted there's fangs sign on patient.Only then we know the cause of the shock.......

2) No antivenom available in disctrict hospital. A patient died of snake bite because no antivenom available. So, we are advised to always check for the antivenom availability in district hospital.

3) We do not know when to give the antivenom, what's the indication for antivenom.

4) Giving a wrong antivenom. A case happened that patient with snake bite died because of wrong antivenom given. The bottle looks like antivenom, name was almost the same too, but actually it was actually not antivenom. So make sure we give the correct antivenom/ read properly the bottle's label before administering to patient.

Some interesting and scary cases shared by fellow ED colleagues during the teaching session:
1) Patient presented with periumbilical pain, then collapsed. ECG showed sinus rhythm to ED doctors and also the medical team. The next day, when the physician came and saw the ECG, he said that might be a MI. So, did another ECG, and true enough, it shows MI changes. But, eventually patient died of cardiogenic shock.
Was my friend's case, too bad she didnt took that ECG picture and show to us what went wrong with 'normal looking' ECG which ended up as MI changes. Gonna find that out later on.

2) An abdominal pain green zone patient, eventually turn out to be triple AAA.

3) A colleague who was a Day 3 HO last time in his first posting in Medical department, was asked to escort a so called "stable" intubated patient to Timberland for a CTB. THat time he was quite free, thus was asked to escort the patient. But, that patient later on asystole at Timberland main door, before went in for CTB. He got panic, called physician, who asked him to called the Timberland physician for help as that physician wasn't at the scene. So then, he called the Timberland physician for help. Resus was successful and patient got the CTB done and return back to SGH. What an eventful day for a Day 3 HO.

And many more cases discussed during teaching session.

Indeed, teaching sessions are fun, everyone get to voice out their own opinion and share their scary and yet interesting cases, but sometimes it felt like time dragging for the PM shift people and the post night ppl cause they have to go to hospital twice a day and the post night ppl cant get back to rest early.
It's enjoying for the double shift and morning shift ppl cause they get to relax during the working hour.

Anyway, it's still fun working in ED aka "A + E Department" aka "All + Everything Department".

Had a good night shift yesterday....Thank God....
Green zone cases all finished by 2am......
Total cases I'd seen in Yellow from 9pm to 8 am - 4 cases.....
Case 1- Viral fever in 6 month old baby

Case 2- ? Pseudofainting....Patient came in for 3rd time within 2 days, first time was sent to Red zone because of GCS 3/15 but all vitals are normal, after a short while, GCS bcm full.2nd time was in the evening, also fainting episode. 3rd time during my shift la! After examination, patient looks like she doesn't wanna wake up, cause her eyelashes are still fluttering, and when we hold up her hands, those hands able to swift away from face, pupils reactive, tears rolling down, and later on, regain full GCS after Ammonnia smell. In the end, I had to clerk all her social history, sien, anyway, Psy team have to sort that problem out in the end.

Case 3- AECOAD secondary to Pneumonia

Case 4- 65 yo male, k/c/o HPT,Gouty arthritis, IGT, most importantly, he had IHD with CABG done in 2004 on life long Aspirin. Presented with strong dizziness, nausea, vomitting, chest pain, abdominal pain, with BP on arrival 186/115. ECG showed got some ST elevation only in V2, and T inversion in V1-V3. So sent patient to Red zone for better monitoring. Serial ECG shows no dynamic changes only ? deeper T inversion. Cardiac enzymes x2 not raised. Referred to Medical team, they also not sure if to treat as ACS. But still loaded him with Aspirin only. In the end, we noted main problem was dizziness, and later, dx as BPPV. Referred to ENT team, thought they will just give outpatient appointment, but instead, they quite unhappy, and said we never rule out the Central cause, asked whether did we do a CTB. WTH? CTB????? with no neurological deficit? So, maybe they had too many BPPV referral, so they were quick enough, to ask to send the patient to ENT clinic TRO BPPV at 7.30am.
When I came back from CME, the patient was discharged by the MO already, and the ENT dx and plans are the same as Medical plans.

Anyway, it's quite a quiet night yesterday. Red zone was totally empty with only one case which I sent in from yellow for ?ACS but was sent out after dx as BPPV.
Was busy with that only case, walk left right, up down for that ?MI case. And my others colleague were sitting there resting. So after settled the case, I just stay in Red zone for a short nap from 6am till 7.30am till a friend called me up from yellow zone asking me to refer that pseudofainting case to PSY and discharge her.

After bought some over night half price buns from Mita bread shop, reached home at 10am, quickly downloaded the latest episode of City hunter. Download speed was good enough to complete downloading not long after I finished my hot bath and breakfast.
Couldn't really concentrate on the series cause too sleepy already but still able to finish it though. haha..

Then started to sleep at 12 pm........ZZZzzzZZZzzz.....till 7.30pm.....woke up and commenced my gastro-resus. My sleeping day, was truly really a sleeping day.....

Heard bout this restaurant quite long time ago but never had chance to try it out until today.....

I ordered this, called 'Chop-Chop-Chop' which has chicken chop, ham, and pork chop....cost RM32

Dinner was nice, had a good chat, from 6.30pm till 10pm.....so much to talk and chat about....about life, work, place, gossips, anything......wish we all were still in the past, in Russia, where study is our only concern......

Case 1: RTA patient came in with cervical spine injury and patient was intubated. CXR shows no pneumohemothorax, further confirmed by Sx team. Later, patient deteriorate suddenly, so found out patient had pneumothorax. Chest TUBE inserted 2-3 times, and finally the third time, there was +++ bubbling. Subsequently, patient proceed to cervical spine CT, which also noted Chest TUBE was actually protruded to lung parenchyma. So, chest TUBE was removed, and inserted again, still not too well functioning. Patient was sent to ICU, and chest TUBE was inserted again and again, 3 times, by anaest in ICU, because of difficulty in insertion, finally the third time, chest tube functioning well. Subsequently, patient did quite well, and slowly recovered, able to be discharged from ICU to a normal ward.

Case 2: A para-suicidal patient, ingested dont know what medicine, query PCM, came to A+E, drowsy, V/S otherwise stable. Ryle's TUBE inserted and gastric lavage with large amount of Charcoal carried out. Suddenly, patient deteriorate, much charcoal came out from her mouth and nostril. Intubation was needed to secure the airway, but it was real hard cause everything we see inside the throat are covered with blackish charcoal, tried a few times by the A+E MO. Anest called in for help in intubation which was successful intubation, and noted by them, actually Ryle's tube was inside the trachea-lungs.

Case 3: BPH patient, came to A+E with hematuria and blot clots noted in CBD and urine bag. Was discharge back home and hematuria is actually blood stained urine. CBD was due off but since patient had hematuria, he needs to keep the CBD and so CBD was changed. Next day, patient came back in again with frank hematuria, it was real maroon blood color ?urine. I think it's blood coming out, not urine anymore.

Lessons to learn:
1) Tubing looks easy, but may cause severe complications.
2) Ask/ consult others always if something isn't fully sure.
3) Dont ever hurt yourself, and come to hospital, cause once you're in hospital, you are our partial experimental material.
So, love yourself, keep healthy, keep yourself away from Hospital.

Every time I see patients coming in to hospital because of so and so complaints, reminds me to be so thankful and grateful to God for what I have and I am now.....

Thank You that I have 4 well functioning limbs,
Thank You for my intact eyes, nose, mouth, ears, skins,
Thank You for my good kidneys, heart, brain and all others internal organs,
Help me to keep myself healthy and well all the time.

Tagging time oso not that long already....

Mum went to meet her old classmates today, one of them is a dermatologist, who also knows acupuncture. According to him, acupuncture is really good in some kind of treatment.
Considering to learn acupuncture next time if it's possible....
Aih....so tired, got to sleep early.........Tomorrow is another busy day ahead....................

CITY HunTER . . . . Lee Min Ho

CIty HuntER...Another KOrean series I'm watching now which its original script was based on the world-famous novel by Japan’s Tsukasa Hojo. Main actor of City Hunter is a real Hot, Tall, Beautiful, Handsome , etc.... named LEE MIN HO......
It's still on air currently, with 2 episodes weekly. Now only aired 8 episodes of 20 episodes altogether. So still have 1 and half month to go to finish this series.

While waiting for the next episodes of the series , and also since i am having 4 days of EOP holidays, i decided to watch another series "Boys over Flowers" by the Main actor - Lee Min HO. Obviously, I am watching it because of Lee Min HO.......

I know all these things aren't realistic, and that's because it's fantasy, everyone tend to submerge themselves into the series,
hoping to live a life just like the series,
hoping to live an eventful life as the series portrayed,
hoping to get rid of usual-working-boring life.
And those everyone, including me as well.

Sometime, I hope I am the actress, because they can experience different kind of lifestyle and different jobs through the characters in the story.... Also able to travel to different places during their work.....But, I have already made my choice to be where I am now, no turning back, no time for regrets, only going forward live my own life......

All these fantasy, series-marathon only can be done during my on-leave time, eventhough I was actually planned to study for my work, but still, I couldn't help it cause fantasy, series are so addicting. Anyhow, my leave has come to an end, so is my fantasy, sigh.....how i wish i can have a life-long holidays.

Another new cycle of HO training starting tomorrow.

Finished the last episode of the HOTTEST korean series titled "49 DAYS"

It's about a young lady, 20+ years old, Shin Ji Hyun was enjoying absolute bliss as she was about to marry her fiancé, Kang Min Ho, but her perfect life is shattered when she gets into a car accident that leaves her in a coma. She is given a second chance at life by a reaper, but it comes with a condition: she has to find three people outside of her family who would cry genuine tears for her. In order to do this, she borrows the body of Yi Kyung, a part-time employee at a convenience store for 49 days. And during these 49 days, she found out that actually her world wasn't that simple and perfect as she thought it would be. The storyline is very interesting and meaningful, teach us to live life to the fullest, life is precious and we should be very thankful, grateful that we are still alive at this moment.

She's the young lady that need to look for the 3 tears in the series, and she really really sweet-looking.......but there's also good-looking actor in the series too....ALso they are so damn good in acting! Had 2 off days plus 1 weekend off, total up 3 days off~
Hooray~ Trying really hard to enjoy my 3 days off to the max! How?

Yesterday, brought mum around to settle her own things, bills and etc.

Today, "Spring cleaning" my own room, trying to clean up all the unnecessary stuffs...
And so, i found this 12 years old 'antique'......

THe letters my old good friends used to post me when i just moved out from my home town...

Now when i read back all the letters, feel like "Wow, we were actually so childish, cant believe that those were the things we talk about at that time"..haha..
I think that 'snail' post lasted for around 1 year then it stopped cause we all busy with PMR, and soon enough, we lost contact till now.....
Everyone has their own life, own career, own family to take care of, and along the way, they found new friends in new society, new environment. Well, that's life...

Felt like throwing away all the "antique" cause they take up a lot of my space, but, i think i should keep it somewhere and let it be my memory....and..
Maybe if I live long enough till 60-70 years old, when i open up this box, those childish gossips in the letters will put a smile on my old face..... =)

LOVE It so much! But i cant own it , cause i dun hav time to take care of them! TOo bad..

Wake up @ 6.30am!!

Finally I can wake up this late to work after long time...
Am in General Med Hematology posting now! One patient to review today and also for discharge.... After that, went to general ward to offer my free time for busy friends there but was ignored cause everything still under their control...so, came back to my own ward, did some reading and eventually fall asleep on the table then woke up again and time to go back !

What time should i wake up tomorrow? 6.30am still? Am oncall tomorrow again....

Went to mount or jungle trekking last Sunday at Mount Santubong! Thank God had off on that day! haha...had lots of fun although it's tiring! Here are some of the pictures my friend captured during the journey!

But his SOS was ignored! haha..after all the encouragement from us, he decided to continue the trekking to the end .....

On my way down to the earth surface, the bottom of the pair of old shoes i used had already 'peeled' off totally and left only the skin to cover my foot! My feet could feel everything i stepped on! It's time to buy a new shoes for next trekking!

After the 6 hours of mountain trekking, everyone went back home with generalized mylagia and mine only recovered fully today! Anyway, had lots lots of fun and lots of thanks to my teamates ! Hope there will be next time but i think lucas wont be joining anymore since he almost gave up this time....

Hmmm.... Time to stop now....Oncall again tomorrow starting from 5 pm..
Before 5pm, what should i do? STudy or NOT study? STuDy.......

Wake up @ 5.30am

Was oncall yesterday, first call in medical department. SO damn busy, admitted around 8 patients. Only able to sleep for 2 hours and that was in female ward call. My friends oncall at the male ward, cant even sleep, did 4 CPR, sent off 2 patients. The worse thing was, they broke one patient's ribs while CPR and causing a flail chest!! Aih, if I am the patient I will decide for DNR. Almost all patients with CPR ended up with poor prognosis.

Whole night oncall running up down taking blood and trace result. Gonna get bilateral knee OA soon. Siennnn...........................

Started with an aspiration pneumonia patient.
Not a bad start though, since my team has only 7 patients compared to the other teams which have 20+ patients. But today is our team admission day, hope it's a peaceful day today.....
Gonna need lots of read-up and catch-up to do..but .....hehehe...

Watched a chinese movie from youtube title 'zhui qiang xi shi'

Last but not least, Jang Geuk Sun's Every little thing star PV...hehe...loves him! =P

Today will be the last day of my 'end of Ortho posting' holidays!
Arggghh......everything has its end point!! Even that, still, I hate that end point the most!

Was having Jang Geun Suk's FEVER for my whole holidays, but started settling down today as my PCM is working hard now thus, will be afebrile by tomorrow because Medical Department tagging is starting tomorrow!!! Sigh....Got to put myself together and start studying for all the common diseases in Medical field!!

Since then, i never opened up my blog due to the busy working life....

Papaya intoxication or Fruit poisoning???

Could you guess what is this?

Does it look like soup? Bee hoon laksa? or Fruit juices? or is that VOMITUS?

Feeling really hungry so came back earlier from Spring today...
There were one medium sized container full of cut papaya. So, i took it and finished all of it...Mum warned me to check if it's spoilt or not. Only yesterday i cut it, so i think it's ok because it was in the fridge. Well, some of the pieces tasted sweet, like papaya, but some tasted a bit sour, so was wondering since when papaya tasted sour? maybe home-plated papaya is a bit different, so i finished all of it because hungry ma...

After papaya dessert, i ate one bowl of 'bee hoon' ..... only then, i felt satiated...

After 2 hours of meal, i felt a little bit of nausea....shit...
What could it be? food poisoning? ate too much papaya? darn...
it has been years since i last vomitted... i didnt wanna break the record now...
The nausea feeling wasnt very strong, it was controllable..
But that feeling doesnt felt good, scared my stomach will be overloaded or overworked to fight the nausea, so i vomitted all the papaya mixed with bee hoon out....
*In fact, i vomitted just to take the picture of the vomitus for writing this post..hahaha..just kidding la....*
feel so much better after i vomitted but i think there were too much gas in my digestive tract which ended up burping repeatedly...

Now i am back to hungry state already...guess, i should let my stomach to rest tonight.....

Or do you think i just eat the vomitus back to stomach?

Finally, i decided to eat the vomitus, since that was already half digested, lets put it back into the stomach to finish the digestion....hahaha..Joking la...
I poured those vomitus into the toilet and flushed it!

Got to sleep early tonight, cant stay up for PPP, aihh...
hope the stomach will have a good rest tonight...Get well soon, stomach!

Opportunity GRABBED...

Like usual, I went to Spring again to hunt for good books to read. While reading for 2 hours, I felt hungry and thus, I walk down to the nearest biscuit stall to buy one big biscuit, which was just enough to fill up my stomach.

While eating, I was watching the function organized by “The Spring”.
Just after I eating, there was another game coming up. The artists, who were invited from Singapore get to choose any audience to be their partner for the game.

The first game was about to start, so I prayed for a ‘brave’ to put up my hand so that I could be chosen by one of the artist. I prayed that, if I have the brave, please let the artist choose me. Unfortunately, I didn’t get chosen even though the artist saw me, and even had an eye contact with me. So, she had chosen another guy up for the first game. So I thought maybe she wanted guys. Feeling a bit down, but anyway, let it be, and to my surprise, it was an acting game! Now, I felt relieved that I didn’t get chosen by her. Hahaha…because I thought the game was about matching up the Chinese phrases.

So, the first game ended, the chosen partner had gotten their own prizes and the second game, also the last game was going to start. This time, it must be the game I was anticipating for. So, I put up my hand again, and wave very hard, and this time, the same artist who saw me just now, she waved back to me and chose me to be her partner!!
When I was getting ready to go on stage, I could hear some of the girls in front of me grumbling for not getting chosen….hehehe…Huahahhaa…..
This time, the prize will be mine for sure, because every contestant will have the prize! Well, not forgetting to thank God. Indeed, He has His own plan, which was why I didn’t get chosen for the first time. Hahaha….
The game went on, there were 3 artists with their own partners forming 3 group of players. Every group has to match as many Chinese phrases as possible. That was the same game carried on yesterday, and the same Chinese phrases of course. So, I sort of familiar with the phrases. No worry. And the result of the game was, every group got to match 7 Chinese phrases. Thus, no winner no loser. Hahaha… Phew…. I thought I will lose…. Wanna know what were the Chinese phrases I managed to match?
1) 口是心非
2) 名不虚传
3) 飞蛾扑火
Hahaha..only 3…I have to pick it up, tear off the double tap, and sticked it on my clothes.
So, I have no time to match all the 7 phrases. I tried my best. The artist managed to match 4 more phrases which gave a total of 7 phrases. Hahaha… After the game, i get to take a group photos with the artists and the other 2 contestants....but i dont have the photos with, photos were taken by the professional photographers..i doubt if those photos will be published in newspaper or not! huahahahaha..hope it will!!! hahaha...

Anyway, I got prize at last! Huahaha…
These are the 2 female artists from Singapore.

One acted in series titled 一切完美, which I watched first few episodes. This series only will be aired in astro this coming month, in fact, my friends already finish watching it! Haha….and now, they even watched the second season of the same series.
Another artist acted in series titled “beach volleyball” if I m not wrong.

I didn’t know what time the function had ended, because as soon I got the prize, I went back to the MPH bookstore, spending my time with my lovely books…….

How’s it tasted? Tasted like mooncake la! Haha…sweet as usual.. Personally, I don’t really like to eat it due to its sweetness. But, since it’s mooncake festival, why not get one for family? Haha…since there’s an opportunities to get it without buying it, no harm trying right?
Hahaha….

Moral value: Grab any available opportunities, eventhough you have no confidence to get it, but at least you tried…most importantly, pray for it! Huahaha…

1 more week of holidays or is it less than 1 week? Or maybe more than 1 week?
Have been counting down the left over holidays i have before i start my job which i dont know when it going to start keep me busy all day long.

During this left over holidays, i have been grounded in the biggest shopping mall i could find in this city. It is fun to be grounded in that mall with its free cooling air con and also free book store where you can read all kinds of books with a condition, you cant bring the book out unles you buying it!

So, in order to keep my wallet filled, I have been going to that book store every single day for reading whatever books they have. It is such a fun to have chance reading all those books. Wish i could stay there too!

Pre-job game

Phew~ I just finished a 600 pages thick of novel titled “Secret” today. This book had taken me almost 12 hours to finish it. Well, it is a good book to read but I am sorry for not remembering the author of the book. *Bow*

While resting my eyes before I got sticked my head into another new book, I look around and saw this paper advertisement glued on the wall of the book store. The paper on the wall is advertising a discount for a group paint ball games. Wow!

Guess I got to take out my paintball equipment and start scanning through if there is any damages needed for repair or any necessary to add some Smart Parts Paintball Guns.

Till then, I got to gather up a group of people for an exciting paintball game before I start my job soon.

Tears stung my eyes and my heart ached as I climbed into my car. My emotions swung wildly from assurance that I had said the right things, to doubt in the stance that I had taken. Was I wrong to share with the group that I feel scripturally responsible to teach my sons from II Corinthians 6 to marry a Christ-follower and be "equally yoked" in marriage? I longed to go back to the offended woman and tell her of my great compassion for my fellow sisters who are married to unbelieving husbands. Instead, I pulled out my cell phone to call a friend for some consolation and advice. It was then that the lesson I had learned from the Bible earlier that day pushed its way to the front of my anxious mind.
I'd read in Exodus that morning about Moses in the midst of a very discouraging situation. After years of mistakes, lessons in the desert and insecurity in his speaking skills, he had finally done exactly what the Lord said. He had gone to Pharaoh and said, "Let my people go." Surely now all the pieces would fall into place. He was at the right time and place, and he had obeyed what the Lord had told him. Moses was about to learn a lesson that many of us are facing: The blessings that follow doing God's will are not always the ones we expect.
He may have expected Pharaoh to get a thoughtful look and say, "Of course you can go, Moses. I know that God is leading you, and we just need to do what He says." That's not what happened, though. Not only did Pharaoh mock God, he doled out even harsher treatment to the Israelites by forcing them to continue to make bricks without the provision of straw. The people were outraged, and they turned on Moses. How discouraging! Things weren't going at all the way Moses had imagined. Now that he was on track with God and walking in obedience, life was supposed to get easier. Right?

All the lessons Moses had learned in the desert shepherding for 40 years served him well now. We see that despite his disappointment, he turned to God in prayer. It was a distressed prayer, but it was real and heart-felt. When he faced an indignant Pharaoh and his angry people, Moses returned to God and poured out his heart. Exodus 5:22-23 tells us, "Moses returned to the Lord and said, 'O Lord, why have you brought trouble upon this people? Is this why you sent me? Ever since I went to Pharaoh to speak in your name, he has brought trouble upon this people, and you have not rescued your people at all.'" (NIV)
When Moses returned to the Lord with the pain that he was feeling, the disappointment in his circumstances, and the confusion about his calling, God met him there. God didn't respond to Moses with anger or rebuke. He answered with a glimpse into future freedom and a rousing message for His people straight from the very heart of God. Moses' struggles didn't end there, but God had given him the encouragement that he needed to forge on toward the calling.

In the car, I set my cell phone down, closed my eyes and bowed my head. I poured out my confusion and disappointment to God, and He met me there. My emotions settled, my mind found rest and God's peace poured over me. He is good! Doing God's will is rarely easy, and the blessings of growth may come wrapped in pain. However, we will find Him to be faithful and His Word sufficient when we return to Him.
Dear Lord, Doing Your will and following You is its own reward. Even though the way may get bumpy as we obey, You are always with us. You are faithful to reassure us and encourage us when we come from the places that our own thoughts take us and return to You. Thank you! In Jesus' Name, Amen.

The Beginning of the WORLD

...did he have to sit down and plan it all out first? (And if he did, where did he sit down? He hadn’t thought up chairs yet!)

Or did He just think it all up at once?

Did God get out a piece of paper and start drawing out ideas - and then scribbling them out when he didn’t like them? Did he try different ideas for things? Like, did God think people might be good with six legs instead of two?

Or, because He is perfect, were all God’s ideas right from the very start?

And how long would it take to think up EVERYTHING in the universe?! Did it take a day? Or a week? Or maybe it took forever. But since forever has no beginning and no end, when he finally said, “That looks good!!” and then created everything, there still was forever left.

Or maybe He thought it all up in an instant!

Something to think about!

One thing is for sure, if it weren’t for God, we wouldn’t be here!

...God created the heavens and the earth.

Before there was anything, there was GOD.

Before there was ice cream and chocolate sauce; or popcorn and pop; or computers, VCRs and TVs; or the sun, the moon and the stars - or even space itself - God was there. There wouldn't be anything at all if God hadn't thought it all up.

He made moose with great big antlers, and little gophers and chipmunks and squirrels. He made monkeys swinging through the trees and giant gorillas sitting in the forests eating bananas. He made huge elephants, and he made hippopotamuses rolling around in the mud. How did God think of all those things?!

What a great imagination God has!

Then God said, "Let us make people in our own image" (God said, "us" because God the Father was there, and so was Jesus, and so was the Holy Spirit - but still he was just one God). So God made people in his own image.

When you look at yourself in a mirror, you see an image of yourself. You see a reflection of what you are like. God made us in his image. We are a reflection of what God is like. When God sees us, he sees a reflection of himself.

You are the image of God. Always remember that.

So God looked at all that he had made, and he saw that it was good.

And there was evening, and there was morning, and that was the sixth day.

On the morning of the seventh day God saw that the whole universe and everything in it was finished. Nothing else needed to be made. And it was all very good.

And so God rested on the seventh day. He celebrated all the good work he had done. God blessed the seventh day and made it holy.

What does that mean?

When something is "holy," that means that it is special. It belongs to God. When God made the seventh day holy, he made it his day. It belongs to him. He made the seventh day a special day, different from all the rest.

God rested from his work on the seventh day and enjoyed all that he had made. That is what God wants us to do too. On the seventh day of every week, God wants us to rest and enjoy all that he has made. That way we will always remember that all good things come from God.

God made everything there is, and he made it to be good. Always remember that.

God is so wonderful!

Chinese Steamed Cake

Mev asked me for a favor, to find a recipe of chinese dessert, and here I am, posting up the chinese dessert recipe, Chinese steamed cake!

6 eggs
1 1/4 cups white sugar
2 1/2 tablespoons water
1 1/2 cups cake flour
1/2 teaspoon baking powder
1 1/2 teaspoons almond extract
1/4 cup confectioners' sugar for dusting

Arrange a large bamboo steamer or a large vegetable steamer over simmering water. Make sure it is large enough to hold the baking pan. Line a 9 inch square pan with waxed paper.
Separate the eggs. Place the yolks in a large bowl along with the sugar and water. Beat with an electric mixer on medium speed until the mixture has increased about three times in volume. Whisk together the flour and the baking powder; sift this over the egg mixture, and fold in gently. Blend in the extract.
Whip the egg whites in a clean bowl to stiff, not dry, peaks. Fold into yolk base. Pour the batter into the prepared pan, and smooth out to edges. Rap the pan on the counter to get rid of large air bubbles.
Place the pan into the steamer. Cover the top of the steamer with a kitchen towel, and place the lid on top; the towel will absorb any steam that collects on top from dripping onto the cake. Steam for about 20 minutes, or until a toothpick inserted into the center comes out clean. Cool on a wire rack. Dust with confectioner's sugar.

Had just finished watching a movie, telling a story about a working girl, who works around the clock to catch her own dreams. Having her own clothing line was a dream she never thought it will come true some day. But what she is working now, as an assistant of a fashion designer, is slowly drawing her near to her long waited dream.

But, in spite of that, she had an obstacle from her dad, who thinks that school is more important for a 15 year old girl. In the midst of that, the girl finally found a solution for her to study in college and working her favourite job at the same time.

New cycle!

Had just finished my immunoclass today. Our new cycle will be about children infections, studying about all the infectious diseases in children, newborn and also fetal!

Classes will be conducted in a newly built hospital but the hospital is quite far away, around 25 minutes bus ride. SO, got to wake up early in the morning to catch the bus, or else, there will be no way to get there.

Hope, it will be an enjoyable classes!

Health Tip: Help Prevent Nosebleeds

If your child has regular nosebleeds, there are precautions you can take to help prevent the problem.

The air in your home may be dry. Use a cool mist humidifier, especially at night, in your child's room.

Don't allow your child to blow the nose too hard, or pick the nose.

Apply a small amount of petroleum jelly to the inside of the child's nose several times a day, and always at bedtime.

Apply saline drops or nasal spray to the nose, as recommended by your pediatrician.
If your child has allergies, seek regular treatment from the child's doctor.

New Technique Sees Into Tissue At Greater Depth, Resolution

By coupling a kicked-up version of microscopy with miniscule particles of gold, Duke University scientists are now able to peer so deep into living tissue that they can see molecules interacting.

If future studies in animal models prove fruitful, the researchers believe that their new approach can have a wide spectrum of clinical applications, from studying the margins of a tumor as it is removed from the body to assessing the effects of anti-cancer agents on the blood vessels that nourish tumors.

The Duke bioengineers combined tightly focused heat with optical coherence tomography (OCT), which has often been called the optical equivalent of ultrasound. OCT is commonly used in medical clinics where imaging at the highest resolution is critical, such as in the retina. These experiments represent the first time the technique has been extended to the functional imaging of cells expressing particular molecular receptors.

"This technique could possibly augment traditional methods of deep-tissue molecular imaging with a relatively high resolution," said Melissa Skala, a postdoctoral fellow working in the laboratory of Joseph Izatt, professor of biomedical engineering in Duke's Pratt School of Engineering. "Not only were we able to get better images, we were able to specifically target the types of cells we were looking for."

The results of the Duke research were posted on line by Nano Letters, a journal published by the American Chemical Society. The research was supported by the National Institutes of Health.

For their experiments, the Duke team attached nanospheres of gold to a targeting molecule known as a monoclonal antibody.

Gold is a metal that not only is an efficient conductor of heat, but whose effects in the body are well known. The antibody they used targets epidermal growth factor receptor (EGFR), a cell-surface receptor implicated in cancer.

These "tagged" antibodies were then applied to the surface of a three-dimensional tissue model composed of human cells – both cancerous and non-cancerous. Skala hoped that these antibodies would home in on cells that were overproducing EGFR on their surfaces, an indicator of cancerous activity. Then the photothermal OCT would be able to detect them by showing where the gold spheres were concentrated.

"When we directed the photothermal OCT at the tissue, we found that the cells that were overexpressing EGFR gave off a signal 300 percent higher than cells with low expressions of EGFR," Skala said.

Adding heat to this form of microscopy technique created a phenomenon much like that seen on very hot days, when portions of the pavement far in the distance seem to float or hover above the road.

"The heat causes a distortion in the way light is reflected off the gold nanospheres in a characteristic way," Skala explained. "As we changed the temperature, the light pathways would change in measurable ways."

In this manner, Skala explained, they were not only able to "see" cells within the tissue, but they were able to capture the molecular function of an antibody attaching to a receptor.

"The use of metal nanoparticles as contrast agents with photothermal OCT technology could lead to a host of potential clinical applications," Izatt said. "Organically-based contrast agents can cause damage or death to the targeted cells, while metal nanospheres are relatively safer."

"Also, given the wide range of nanoparticle shapes and sizes, coupled with the ability to 'tune" the optical wavelength of the OCT, we can customize our approach to many different target types," Izatt said.

Skala plans to expand the use of this approach in animal models to better understand the role of different cancer therapies. Tumors with elevated levels of EGFR are known to have a poor prognosis, and she plans to use photothermal OCT to measure how these tumor types react to different therapies.

Other members of the Duke team were Adam Wax, professor of biomedical engineering and his graduate student Matthew Crow. Skala also worked with Mark Dewhirst, a cancer researcher at Duke University Medical Center, and plans further collaborations with the Dewhirst laboratory to apply this technique better understand the fundamentals of cancer.

Just had a basketball trial yesterday... and as a result of it, i was diagnosed with DOMS.. which is Delayed onset muscle soreness....

What is DOMS?
Delayed onset muscle soreness (DOMS) describes a phenomenon of muscle pain, soreness or stiffness that is felt 12-48 hours after exercise, particularly at the beginning of a new an exercise program, after a change in sports activities, or after a dramatic increase in the duration or intensity of exercise.
This muscle pain is a normal response to unusual exertion and is part of an adaptation process that leads to greater stamina and strength as the muscles recover and build hypertrophy).

This sort of muscle pain is not quite the same as the muscle pain or fatigue you experience during exercise. This delayed pain is also very different than the acute, sudden pain of and injury such as muscle strains and sprains, which is marked by an abrupt, specific and sudden pain that occurs during activity and often causes swelling or bruising.

The delayed soreness of DOMS is generally at its worst within the first 2 days following the activity and subsides over the next few days.

Delayed onset muscle soreness is quite common and quite annoying, particularly for those beginning an exercise program or adding new activities. A beginning exerciser who bikes 10 miles, followed by push-ups and sit-ups is likely to experience muscle pain and soreness in the next day or two.

Causes of DOMS
Delayed onset muscle soreness is thought to be a result of microscopic tearing of the muscle fibers. The amount of tearing (and soreness) depends on how hard and how long you exercise and what type of exercise you do. Any movement you aren't used to can lead to DOMS, but eccentric muscle contractions (movements that cause muscle to forcefully contract while it lengthens) seem to cause the most soreness.

Examples of eccentric muscle contractions include going down stairs, running downhill, lowering weights and the downward motion of squats and push-ups. In addition to small muscle tears there can be associated swelling in a muscle which may contribute to soreness.

Is there any treatment for DOMS?
There is no one simple way to treat delayed onset muscle soreness. In fact, there has been an ongoing debate about both the cause and treatment of DOMS. In the past, gentle stretching was one of the recommended ways to reduce exercise related muscle soreness, but a study by Australian researchers published in 2007 found that stretching is not effective in avoiding muscle soreness.

So does anything work to reduce delayed-onset muscle soreness? Nothing is proven effective, but some people have found the following advice helpful, but it's best for an individual to try a few things to see what works for them. Ultimately, best advice for treating DOMS is to prevent it in the first place.

Here are some tips for dealing with soreness after exercise:

* Wait. Soreness will go away in 3 to 7 days with no special treatment.

* Try an Ice Bath or Contrast Water Bath. Although no clear evidence proves they are effective, many pro athletes use them and claim they work to reduce soreness.

* Use active recovery techniques. This strategy does have some support in the research. Perform some easy low-impact aerobic exercise to increase blood flow. This may help diminish muscle soreness.

* Use the RICE method of treating injuries.

* Although research doesn't find gentle stretching reduces soreness, some people find it simply feels good.

* Gently massage the affected muscles. Some research has found that massage was effective in alleviating DOMS by approximately 30% and reducing swelling, but it had no effects on muscle function.

* Try using a nonsteroidal anti-inflammatory medication (aspirin or ibuprofen) to reduce the soreness temporarily, though they won't actually speed healing.

* There is some evidence that performing Yoga may reduce DOMS.

* Avoid any vigorous activity that increases pain.

* Allow the soreness to subside thoroughly before performing any vigorous exercise.

* Don't forget to warm up completely before your next exercise session. There is some research that supports that a warm-up performed immediately prior to unaccustomed eccentric exercise produces small reductions in delayed-onset muscle soreness (but cool-down performed after exercise does not).

* ** If your pain persists longer than about 7 days or increases despite these measures, consult your physician.

* Learn something from the experience! Use prevention first.

Prevention
While DOMS is common and annoying, it is not a necessary part of exercise. There are many things you can do to prevent, avoid and shorten the duration of DOMS:

* Warm up thoroughly before activity and cool down completely afterward.
* Cool Down with gentle stretching after exercise.
* Follow the Ten Percent Rule. When beginning a new activity start gradually and build up your time and intensity no more than ten percent per week.
* Know the 10 Tips for Safe Workouts.
* Follow the Spring Training Fitness Tips.
* Hire a Personal Trainer if you aren't sure how to start a workout program that is safe and effective.
* Start a new weight lifting routine with light weights and high reps (10-12) and gradually increase the amount you lift over several weeks.
* Avoid making sudden major changes in the type of exercise you do.
* Avoid making sudden major changes in the amount of time that you exercise.

Certain muscle pain or soreness can be a sign of a serious injury. If your muscle soreness does not get better within a week consult your physician.

今天早晨太迟醒, 约了朋友 7:15早上去医院报道, 可是已经7:40 了, 我还在梦中, 直到朋友打电话 call 我, 我才被吓醒。告诉他，他们先去，我自己慢点到。挂掉电话后，想起妈在洗衣，不想让她匆匆忙忙的载我去医院，于是就拿起手机联络我那个朋友，请他来我家载我去。过后就很快得换好衣，拿了包包，就去附近的巴士站等那朋友的到来。等了等，怎么那么久，还没来呢？不可能那么慢。心想可能他已经先去了。拿起手机，真的不出所料，手机有个SMS，说他们需要先去了因为别的朋友在等着他手上的信。

本来想走回去，叫妈载，想一想，家的篱笆已经关上了，难道还要麻烦妈出来开，然后在匆匆忙忙的载我去医院吗？

于是决定要坐巴士去医院，可是问题是，哪一辆是去医院的呢？

真的感谢主，刚好有个女子刚到巴士站。我就鼓起勇气，礼貌性的问她哪一辆巴士可以抵达医院。她也很友好的告诉我，只要是14号都能到医院。

我的眼睛就一直望着远方来的车，心里默默的祈求上帝，那14号巴士快点来。

不久后，有一辆14B 巴士就出现了。好开心！终于等到了！还以为要等很久呢！

很开心的上了巴士，这是我在这里第一次上巴士。

一坐下，眼睛就不停的东张西望，巴士里只有10个人！好空的巴士！

她用英文说：One thirty （RM1.30）。

我就从我钱包拿出两块钱给她。

这就是 bus ticket。Ticket 上写着80 cents。其实有两张 bus ticket。可是另外一张飞掉，所以只剩一张。另外一张写着 50 cents。所以中共 RM 1.30。嘿嘿嘿。。。。

还好钱过后，眼睛就望出窗口，看看早晨的风景，看到的都是车，好多车，塞满了路上。心想，可能会很迟才到医院。哪知，巴士还不赖，不到30分钟，我就在医院的巴士站。很开心的下了巴士，走进医院寻找那几个抛弃我的朋友。哈哈哈！开玩笑的啦！我还要感谢他们呢，如果不是因为他们，我也不会有怎么好玩的搭巴士的回忆！

RM1.30 的巴士费, 便宜过我自己驾车去吗? 算一算……..

Brother came back yesterday evening, brought back a big bag of coins which he collected. Mum asked him to bring it back and change into notes. SO, 3 of us sat down together and started to count the coins. Before we started, we had to guess how much is it altogether?

Day 2 in London

Phew… finally uploaded all the pictures Johnny and I took today….

Now is the time to write about it~ haha…

My first day (23/6/08) trip in london started at 9.00 am. Lynn wei and her cousin, Shirlyn brought us a supermarket to withdraw our money from the atm machine.

We came out from lynn wei’s aunt’s house…

Opposite her house is a small garden…

The flowers in the garden…

We walk down the street to a shopping mall to withdraw the money….

On the way, we passed by this classic old church…

In the shopping mall now…. see the maroon colored plant? According to lynn wei’s aunt, those plants are the pink colored “mei hua” which will only blossom during winter like those in China….

After we withdrew our money, we stop by this supermarket called Tesco metro…this is an international supermarket with all the cheap grocery….

After that we separated into 2 groups… lynn wei and shirlyn, her cousin, went to other places..while us, Johnny, Adeline, Khian Lu, Bryan and I, went to the Buckingham palace…

We are living in Ealing Broadway (the red circle), and now we are going to Victoria….

This is the route we walk…. SO, we started from the Victoria Metro…and walk till the Buckingham palace to see the changing of the guard….

Came out from the Victoria, we saw this tourism booth…. Took a map from there…. and continue our walk to the palace…

Passed by this Ruben’s hotel… opposite this hotel is the Royal mews…

Almost there now…just walk straight then turn left… follow the crowd, and you will reach there….

Soooooo many people..haha…

After waited for 10-15 minutes…till 11.30 am.. the marching of guard started…. a troop of band passing by… there were 3 troops of guard… this only one of them… more picture in johnny’s blog….

All the guards marched into the palace…and the 2 bands will perform a few songs..until 12 pm…they marched out from the palace …

After that, the gate were closed and that’s the end… everyone leave the place as well…
Went there just to watch the marching… once in a life time is enough …..haha…
The changing of the guard is on everyday at 11.30 am in summer but in winter time, only certain days…
We waited there for so long, under the hot sun just to watch the marching……
Lynn wei said..last time during winter, when she went there, it was so freezingly cold…. But that time, there wasn’t so many people as summer…

Took a map of the place… next destination will be the St Jame’s park…. A very nice, green and beautiful park.. also, there were a lot of people…

Walk along the lake….

Along it, you may see a lot of ducks and big white bird species… don’t what is it called…

There is a board here about the species of the bird present in the lake…. So check it out yourself..ahaha

I picked up some of the dropped bread crump and try to feed the duck too….haha…

Way exit form the park..The flowers are very pretty…. The gardeners must have done their very good job to maintain the all the plants…

At the exit of the park, there’s a memorial, commemorates all those members of the household division who died in the second world war and in the service of their country since 1918…

After the st james park, we headed to the house of parliaments…

ON the way to the parliaments, passed by this memorial wall, with a big ball in front..this wall was in front of the Churchhill museum. We didn’t enter the museum cause it is not free…haha… 9.30 pounds for student ticket… adult will be 12 pound… quite expensive…so we just continue on to the parliaments..

Second one..u guess what is it? Haha…it is the policeman’s horses’ feces…. Very smelly ah!! Haha..

A clock with the name BIG BEN…..is a part of the houses of parliaments….you can see ..it was 1 pm that time…haha….

There is no pedestrian cross on the bridge…so in order to cross the road, we have to take the underground route…

After crossing the road, we continue our river front walk till the London Eye….

Sat down at the river front for a small break…and replied the tourists’ waves in the duck boat…
Why is it duck boat? Duck, because the boat can drive on land, and sail in the water..just like a duck… didn’t manage to see it on land…haha..

Here is the London Eye…. A lot of people too….one of the MUST DO THINGS in london…
You have to buy the ticket to ride inside the small oval shaped thingy called CAPSULE…
Johnny wanted to ride in it, but sadly, nobody want to accompany him….. haha…so at last, he didn’t get into the capsule…

Before you enter the capsule, you got to go through a human-scanned and then join the long queue to enter the capsule…
Anyway, we didn’t get to join the queue, so we continue our river front walk…. The walk from London Eye was every interesting because there are many people selling-arts…

Is this a real human or a statue? How do u think? Haha..it is a real man painted with silver color and put on with the silver colored costum…

This is the Trafalgar square…. Behind it is the National gallery..

National gallery with a big fountain in front..The national gallery entrance is free of charge, so we decided to have a few glance of the gallery…. The gallery is very big, u have to search for the sign board all the time, if not, you wont know where is the exit. Inside, the arts are divided according to years of their production. From 1500-1600 till 1800-1900. A lot of people inside too…

Look down from the national gallery… saw the flags on the top of those buildings in front…those building surroundings are the embassies of different countries..

What are these people doing in front of the gallery? I think they were promoting the London zoo…

This is the Charing cross street heading to the Oxford street… heard the stuffies in Oxford street are cheap… there are lots of sale, but when u convert it to Rm, it become very expensive..ahaha…so only window shopping…

This is the Oxford street…lots of transports and people…After the oxford street, we went straight till the Oxford circus metro…to take a metro back to Ealing broadway, the place we living.

After we reached Ealing broadway, we went back to the TEsco supermarket to shop for our dinner, tomorrow breakfast and lunch…..

This is what I bought…Bread- 37pence, fresh milk-1.56pd, sun lotion-4.99pd, mineral water-77pence, cheese spreads 2 for 1.50 pd.. and another maltloaf which I had already eaten, for 20 pence…

After everyone finished buying all their stuffies, we walk back to lw’s aunt’s home…..
Back at home at around 7pm……

Weeeee!! Some update for today! Went to skating with friends this evening! Damn FUN!! hahaha..
This time was the second time i went for skating....there's a skating ring (field) near to our hostel...entrance fee is 50 roubles......i borrowed a pair of skates shoes from a girl in this hostel...very nice shoes...

The skating ring is not open every day....so when it's opened, a friend will inform us...then we will go there and have fun! Last few days the ring was closed...today it was on! so we got to grab this long waited opportunity to have fun liao......Went there around 4.45 pm..

Not many people skating....around 20 to 30 people....a lot of kids...small kids are skating and a few teenagers....some of them are very pro! and even the kids too ....A kid saw me fall down so many times , finally he couldn't stand it and taught me how to skate better.....how? You have to bend your body forward a bit to have a better balance! hahaha.....

I got to buy myself a pair of DC Skate Shoes since I am going to use it so often. Hehe…

Wake

Finally I reached London after a 4-hour flight from Moscow. We reached London at around 6.20pm. Lynn wei helped us to buy a train ticket, actually it is a underground metro ticket. Cost us 3 pounds per ticket. We took the train and got out from a stop to change to another line to reach lynn wei’s aunt’s house. The hardest things during the journey back to her aunt’s house is carrying the 23 kg of my luggage up to the stairs. SO tiring!! Anyway, we got back to her aunt’s house safely. Haha…

Thank to her aunt for the wonderful dinner. Took our bath after the dinner and we all sat down on our computer to surf internet! Haha..thanks to her aunt’s for the wireless internet. The internet service is very good and fast!

Hop on to a website during the surfing. This website provide a huge collection of Liquid Force Wakeboards. All this collections are specially needed during summer at such a time like this!

The turtles

A turtle family decided to go on a picnic. The turtles, being naturally slow about things, took seven years to prepare for their outing. Finally the turtle family left home looking for a suitable place. During the second year of their journey they found a place ideal for them at last!

For about six months they cleaned the area, unpacked the picnic basket, and completed the arrangements. Then they discovered they had forgotten the salt. A picnic without salt would be a disaster, they all agreed. After a lengthy discussion, the youngest turtle was chosen to retrieve the salt from home. Although he was the fastest of the slow moving turtles, the little turtle whined, cried, and wobbled in his shell. He agreed to go on one condition: that no one would eat until he returned. The family consented and the little turtle left.

Three years passed and the little turtle had not returned. Five years...six years... then on the seventh year of his absence, the oldest turtle could no longer contain his hunger. He announced that he was going to eat and begun to unwrap a sandwich.
At that point the little turtle suddenly popped out from behind a tree shouting, 'See! I knew you wouldn't wait. Now I am not going to go get the salt.'

Phew~ Just finished my second last exam today. It was a very tough exam, which covers lots lots of information. The exam was in oral form. There were 3 teachers in a classroom and students went in one by one, choose their own card which contains 3 questions, one set of analysis including ECG, blood analysis, urine analysis, biochemical analysis and an X-ray. After taking your own card, sit down, and prepare for the answers and wait for the teacher to call your name. This was how the exam conducted.

The exam started at 9 am and you can go back as soon as you finish answering the teacher. I finished answering at around 1 pm. My questions weren’t hard for me. I got the questions I know how to answer. Thank God. Hahaha..

After the exam, came back home and start online!! I had fasted for online to study for my exam and now I can surf as much as I want. It is summer now, so I was searching for shorts to buy and I found this 511 Tactical Shorts very nice. I am going to buy it now!

Like I said before, I am going to Boston to visit a friend of mine during the summer. And I had done my homework and research about Boston. Found a lot of Things to do in Boston.

Chocolate Coffee Cheesecake

A chocolate coffee cheesecake... the cake i wanted to bake to pay my debts to the ones that i owed. hahaha... see la when i am free, i bake ... now just let you see the recipe first!

3 cups crushed chocolate graham crackers (about 20 sheets) (I used 2 cups)
1/2 cup butter or margarine, melted
PAM Original No-Stick Cooking Spray
4 (8-ounce) packages PHILADELPHIA Cream Cheese, softened (I used 3 blocks, 2 blocks of 1/3 cream cheese and 1 block normal cream cheese)
1 cup DOMINO Granulated Sugar (used 3/4 cup sugar)
1/4 cup coffee liqueur
1 teaspoon instant coffee granules
1 teaspoon vanilla extract
4 large eggs (used 3 eggs)
4 (1-ounce) bittersweet baking chocolate squares (used 3 ounce)
Mocha Sauce (omitted)

1. Stir together crushed graham crackers and butter; press mixture into bottom and up sides of a 9-inch springform pan coated with cooking spray.
2. Bake at 350° for 10 minutes. Cool crust in pan on a wire rack. Reduce oven temperature to 325°.

3. Beat cream cheese and 1 cup sugar at medium speed with an electric mixer until blended. Add coffee liqueur, coffee granules, and 1 teaspoon vanilla, beating at low speed until well blended. Add eggs, 1 at a time, beating just until yellow disappears after each addition.

4. Remove and reserve 1 cup cream cheese mixture. Pour remaining batter into prepared crust.

5. Microwave chocolate squares in a medium-size glass bowl 1 minute or until melted, stirring after 30 seconds; let cool slightly. Stir reserved 1 cup cream cheese mixture into melted chocolate, blending well. Spoon chocolate mixture in lines on top of batter in springform pan; gently swirl with a knife.

6. Bake at 325° for 1 hour or until almost set. Turn oven off. Let cheesecake stand in oven, with door closed, 30 minutes. Remove cheesecake from oven, and gently run a knife around outer edge of cheesecake to loosen from sides of pan. (Do not remove sides of pan.) Cool on a wire rack. Cover and chill at least 4 hours.

7. Remove sides of springform pan. Serve with Mocha Sauce.

Nowadays, you can hardly see girls wearing skirt. Most of the time, girls prefer jeans than skirts because jeans are more comfortable especially when you are rushing to somewhere or taking public transportation to some where.

In my university, I can see 95% of the students are wearing jeans or pants almost everyday including myself. Jeans or pants are everyone’s favourite because you don’t need to worry what color or which style of top to match with jeans because anything with jeans is always fine and perfect! Personally, I prefer Dockers pants because it is just the type that I like and most important is, it is comfortable to wear and it last for years!

Remembered that when I was form four, secondary school, my brother brought a very unique and nice watch back home. The watch was carved with a cute fish bone on it and bright green color background which blink under the bright light. I liked it so much that I kept on begging my brother to give me the watch. After 1 minute of begging, he decided to give me the watch. Why? So that he had another reason to buy another new watch. What a cunning idea! Anyway, I still got my beloved watch. Till now, the watch is still serving me faithfully.

But unfortunately, last few days, I saw this attractive concord watch displayed in a very high class watch shop.

Phew… Finally another weekend had came. This week had been a busy week. Now, finally, it was time for resting, to have fun with friends, playing games, surfing net, sleep as much as I can to get back all my sleeping time and cook something nice for myself, time to go shopping to re-fill my own foods bank and so on.

A friend of mine had introduced me a good Horror movie, from Frontiers. So, tonight we might going to watch this horror movies. Got to get ready the pop corns, a few small or big pillows, a pairs of good speakers, wide screen laptop (because we don’t have television here) and gather as many friends as possible and watch together in a dark room. Exciting!!

But before that, let me go and watch the trailer in the website first. Can’t wait to know what is the horror movies about.
